Outreach Worker – Part-time

 

Hands to Help Ministries seeks a part-time outreach case worker with a background or training in Social Work and experience with street outreach to work with homeless people on the streets and others in crisis who are at-risk of losing their housing in the Portage Park/Irving Park neighborhoods around Six Corners on the northwest side of Chicago. This person will initiate contact, build relationships, determine needs, create budget proposals for client financial assistance, refer and connect clients to appropriate help, follow-up, and maintain communication with the Hands to Help board.

 

Contract position of 10-12 hours per week at $15/hr.

Outreach Worker

Essential Functions:

1.         Engage people who are homeless in the area and create a working relationship through observation and interactions.

2.         Document interactions with clients through case notes, budgets and action plans to set goals with clients and track progress in helping clients to exit homelessness or prevent homelessness.

3.         Make referrals and help clients to follow through on referrals to social service agencies as needed to assist with healthcare, housing, disability applications, mental health care and other services that may be needed for living.  A binder of resources will be provided.

4.         Create budget proposals with clients to request financial assistance from board for items such as rental or utility assistance.

5.         The Outreach Worker is expected to spend 4 - 6 hours/wk in the field engaging and following up with clients, 2-4 hours/wk meeting with clients at a church office, 1-2 hours/wk of administrative work, including documenting interactions by keeping a daily log of clients along with referrals given. The Worker will meet monthly with his/her supervisor and provide a monthly summary of client interactions and referrals.  An intake form and client contracts will be provided by Hands to Help.

6.         The Outreach Worker will set hours at an agreed upon day and time frame and be present and available in the Six Corners area at that time.

7.         The Outreach Worker will have the Hands to Help cell phone to make calls in the field for referrals or follow-up with clients.  Answering the cell phone during working hours is also expected.

8.         The Outreach Worker would commit to at least one year of work with a review after 90 days.
